6 Lessons to Make the Most Out of Your Dot Phrases

Lesson 1. First, let’s learn about inline search. It’s almost impossible to memorize all the 1,000+ dot phrases available. By using inline search you can look up keywords to easily find them. Inline search:

Lesson 2. The main template we recommend you use in your progress note is the “no-touch” dot phrase (everything documented can be gathered from entering the room, saying hi to the patient and shaking his or her hand.)  This template covers nine organ systems or elements required to be documented for billing purposes. Try to start with the “no-touch” template and then add elements of the physical exam pertinent to the visit.

The yearly physical is sometimes overlooked because it requires extra documentation. This dot phrase will help expedite your progress note and capture the essentials of a good annual physical with health maintenance recommendations from the USPTF. You can also find a dot phrase for a Medicare wellness exam, which has even more documentation requirements. Note that we used the dot phrase (PE adult short CV) to document lung and heart auscultation. We also have health maintenance based on age and sex for point-of-care recommendations.
